Medical Disclaimer

“Client” understands that the information given by “Coach” is not meant to treat, cure, or take place of any treatment, medication, or physician’s instructions or advice, therapy or behavioral health. Client understands that they are ultimately the sole person responsible for their health, food choices, allergies, food intolerances, and actions. Client understands that they should always consult their physician or another healthcare provider first before starting their or any health, wellness, or nutrition program. Client understands that Coach’s support is not a substitute for obtaining professional medical advice from a physician, therapist, or behavioral health specialist.
 

Confidentiality

Coach agrees to keep Client’s information confidential except in those situations where such

confidentiality and personal information added or shared with a third party software provider(s) might be access to help Coach perform their job or for the safety of Clien. Coach will maintain, store, and dispose of any records; including electronic files and communications created during my coaching engagements in a manner that promotes confidentiality, security/privacy, and complies with any applicable laws, regulations, and agreements.
 

NBC-HWC Scope of Practice

Coach understands the relationship between Coach and Client is professional as outlined  by the National Board for Health and Wellness Coaching Scope of Practice:

“Health and wellness coaches work with individuals and groups in the client-centered process to facilitate and empower the client to develop and achieve self-determined goals related to health and wellness. Coaches support clients in mobilizing internal strengths and external resources, and in developing self-management strategies for making sustainable, healthy lifestyle, and behavior changes. While health and wellness coaches per se do not diagnose conditions, prescribe treatments, or provide psychological therapeutic interventions, they may provide expert guidance in areas in which they hold active, nationally recognized credentials, and may offer resources from nationally recognized authorities such as those referenced in NBHWC’s Content Outline with Resources. As partners and facilitators, health and wellness coaches support their clients in achieving health goals and behavioral change based on their client's goals and consistent with treatment plans as prescribed by individual clients’ professional healthcare providers. Coaches assist to use their insight, personal strengths and resources, goal setting, action steps, and accountability toward healthy lifestyle change.

 

NBHWC Pledge of Ethics:

As a Health and Wellness Coach, I acknowledge and agree to honor my ethical and legal obligations to my coaching clients and sponsors, colleagues, and to the public at large, I pledge to comply with the NBHWC Code of Ethics and to practice these standards with those whom I coach, teach, mentor, or supervise.
